EXCLUSIVE: Planned Parenthood May Be Committing Fraud To Hide Transgender Procedures, Watchdog Says

A medical watchdog group is asking the Department of Health and Human Services to investigate Planned Parenthood’s medical coding practices, which they say could point to fraud.

Do No Harm says Planned Parenthood affiliates may have used improper billing codes when prescribing cross-sex hormones to gender-confused young people, according to a new investigation.

“Planned Parenthood affiliates may be deliberately miscoding services to mask their true purpose and to facilitate the improper reimbursement of cross-sex hormone treatments,” Do No Harm wrote in a letter to HHS Inspector General Thomas Bell that was exclusively reviewed by The Daily Wire.

The watchdog group analyzed medical diagnosis codes used by Planned Parenthood affiliates on billing claims for cross-sex hormone treatments and discovered “potential coding discrepancies.” Do No Harm says it found evidence that some Planned Parenthood affiliates have encouraged “strategically miscoding medical encounters” when billing for transgender medical interventions as a means of obtaining insurance reimbursement.

“Many regional Planned Parenthood affiliates openly advertise a practice of facilitating insurance reimbursement by strategically miscoding medical encounters for gender-related mental disorders,” the letter read. “In representing that they use inaccurate diagnosis codes to ‘meet the needs of most insurance companies,’ these affiliates’ public statements reflect well-known strategies for evading insurance scrutiny via intentional miscoding.”

Do No Harm’s investigation was prompted by concerns that Planned Parenthood — one of the largest providers of so-called “gender-affirming” hormones in America — could be “miscoding claims at scale.”

“The transgender industry has become a lucrative business with expensive treatments and long-term patients,” the group writes. “Providers of sex-rejecting procedures are aware of this. They are also aware that health insurance providers — including government providers such as Medicare, Medicaid, and TRICARE — only reimburse for treatments that are ‘medically necessary,’ and that many kinds of sex-rejecting procedures do not fit that requirement.”

This investigation comes on the heels of an HHS report scrutinizing billing practices within the transgender medical industry. The report found evidence suggesting transgender medicine providers across the United States have deliberately used fraudulent billing codes to obtain insurance coverage for procedures such as puberty blockers, cross-sex hormones, and mastectomy surgery. Groups like the World Professional Association of Transgender Health have encouraged such fraudulent billing practices, the report says.

Insurers require procedures to be “medically necessary” to receive insurance coverage. Do No Harm says many sex-rejecting procedures being covered by insurers “do not fit the bill” of medical necessity and could be denied reimbursement — making the transgender medical industry ripe for billing fraud.

“Many types of sex-rejecting procedures are (at best) experimental medicine that pose a ‘risk of significant harms’ with only speculative benefits,” the letter reads. “Because providers know that treatments are reimbursed only when medically necessary, some providers who seek reimbursement for sex-rejecting procedures may deliberately use incorrect diagnosis codes in patient records and on billing claims,” states Do No Harm in the letter to HHS.

Do No Harm worked with a professional data broker to “collect claims-related information in connection with hormone treatments prescribed by Planned Parenthood affiliates” between 2015 and 2025. The analyzed data included diagnosis codes from medical claims used for office visits and hormone treatment prescriptions “where the prescriber is also associated with Planned Parenthood Affiliates.” Do No Harm evaluated medical diagnosis codes for Planned Parenthood office visits that occurred 30 days prior to a claim for a hormone prescription where “the prescriber is also associated with Planned Parenthood affiliates.”  In analyzing this data, Do No Harm found 800 Planned Parenthood patients ages 22 and younger from across the country who appeared to be given hormone prescriptions related to gender dysphoria without proper billing codes.

“Do No Harm and the data broker ultimately isolated a subset of approximately 800 patients from across the country for whom Planned Parenthood affiliates wrote between 1,100 and 1,200 prescriptions seemingly related to gender dysphoria while potentially failing to use the correct diagnosis codes for the associated office visit,” states the letter.

Do No Harm notes there are limits to its investigation and says further investigation from the federal government is needed.

“An investigation by HHS is warranted because Do No Harm’s own investigation, while valuable, has limitations that are inherent in a private actor’s attempt to root out another private actor’s wrongdoing without any of the government’s tools,” states the Do No Harm letter. “We, thus, respectfully recommend that the government further investigate Planned Parenthood’s potential miscoding scheme.”

Planned Parenthood did not respond to The Daily Wire’s request for comment.
